Specialist Prescribing Pharmacist (Band 8a)

Job Summary : The successful candidate will be responsible as the Specialist Prescribing Pharmacist , for leading the delivery of proactive preventative pharmaceutical interventions for moderately frail patients accessing the Staying Well pathway in East, South East and South West Staffordshire. The role will involve assessment and treatment of patients in a variety of community settings including patients own homes, and clinic style multi-disciplinary team consultations. They will lead on the development of systems of practice to support accurate and timely interventions with a focus on prescribing or de-prescribing of medicines to promote positive and proactive impact on patient care to slow down the onset of frailty or reduce its progression. The role will involve supervision, support and advice to other professionals across the health economy and members of the multidisciplinary team as well as support for Foundation Year Trainee pharmacists. Working Hours: The service is operational Monday to Friday 09:00 – 17:00
