Specialist Pharmacist – Anti-infectives, Virtual Ward Service

An new and exciting opportunity has arisen for an experienced pharmacist to join the innovative and friendly Anti-infectives Pharmacy Team at Northwick Park Hospital. We are looking for an individual with experience and a passion in anti-infectives as well as working within hospital pharmacy. The majority of your role will be dedicated to the Virtual Ward and Early Supported Discharge service for the acute infection pathway but will also include working alongside other members of the Pharmacy Anti-infective Team to provide a service to the infectious disease department and carrying out the Trusts antimicrobial stewardship initiatives.
  • To lead, deliver, develop, and evaluate clinical pharmacy services to the virtual ward and early supported discharge (ESD) service for the acute infection pathway in accordance with the objectives and priorities agreed between the post holder, Chief Pharmacist, Lead Anti-infective Pharmacist and the leads for the virtual ward and ESD service, in order to ensure safe, clinically effective, and cost efficient use of outpatient parenteral antimicrobial therapy (OPAT) and other anti-infective therapy including prescribing services when required.
  • To provide a consistently highly specialist pharmaceutical service for patients seen within the virtual ward and ESD service on the acute infection pathway in line with Trust and national standards in order to meet all Clinical Governance requirements.
  • To lead on Medicines Management for the virtual ward and early supported discharge (ESD) service for patients on the acute infection pathway.
