Specialist Clinical Pharmacist, Renal and Frailty

The role involves working closely with The Advanced Pharmacists. In renal this involves providing the clinic Transplant and Tolvaptan clinic supporting the nephrologist across Northampton General Hospital and Kettering General Hospital. Working with the antimicrobial advanced pharmacist you will be providing antimicrobial stewardship across the Trust, supporting RCA, C. diff ward rounds and diabetic foot rounds. As a guide, the split of the post is expected to be: Specialist Pharmacist Renal 0.5 wte Specialist Pharmacist Frailty 0.5 wte You will also provide a patient focused Clinical Pharmacy Ward Service and contribute to the delivery of the Trust’s Medicines Optimisation Strategy
