Full Job Description
We are looking for a qualified independent prescribing pharmacist to join our friendly and supportive Somerset practice and expand our multi-disciplinary team. The post is for up to 37.5 hours per week – applications from candidates seeking part time hours are welcome. Salary will be dependent on experience.
You will be an enthusiastic, caring and motivated professional with excellent clinical, organisational and communication skills, who has the ability to work autonomously but also be a key member of the growing multidisciplinary team at the practice.
Main duties of the job
Your role will be to support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This includes patient medication reviews, helping the practice achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care.

Burnham and Berrow Medical Centre is a large, well established practice with an on-site pharmacy, which provides a wide range of services to around 15,000 patients. We are a friendly practice in a seaside town location and our transport links are great – The M5, Bristol Airport and train routes to London mean that Burnham-on-Sea is very conveniently located.

JOB SUMMARY
To provide Pharmacy services within the practice team based at Burnham and Berrow Medical Centre.
To support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This includes patient medication reviews, helping the practice achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care.
MAIN DUTIES AND RESPONSIBILITIES
Provide sessional clinical support to as an independent pharmacist prescriber (ideally), seeing patients and leading on medicines optimisation.
Support the surgery to improve medication reviews, medicines optimisation and formulary adherence.
Actively participate in regular weekly care home multi-disciplinary team meetings, providing advice on appropriate medications and de-prescribing where appropriate, as well as ad-hoc support to the care home teams.
Support the practice clinical team to authorise patient medication requests.
Support Burnham and Berrow Medical Centre and Somerset CCG plans to reduce medicines waste in primary care.
Interpret and provide complex medicines information to patients, carers and healthcare professionals.
To provide information, advice and guidance on the safe, rational, effective and economical use of medicines.
Undertake clinical audit and other outcome measures to support patient care and safety.
Create and implement medicine related guidelines, policies and SOPs as appropriate to the different service areas.
Provide and receive complex, sensitive information and communication relating to pharmaceutical care of patients.
Provide prescribing service within sphere of competency to patients in primary care, as pharmacist independent prescriber.
To develop, advise, agree and implement action plans at a practice level, monitor the effect of changes made and ensure that any benefits are maintained in the longer term. This will include:
- Reviewing prescribing data for the practice to highlight areas where there is scope for improving the clinical effectiveness of prescribing.
- Obtaining agreement with the practice to implement medicines optimisation projects with the overall aim of following national prescribing guidance and cost effective prescribing.
- Advising GPs and other health professionals on cost effective prescribing and aiding rationalisation of drug choice via formulary/guideline/protocol development.
- At least 2 years post-registration pharmacy experience
- Clinical Pharmacy experience
- Clinical audit experience
- Knowledge and understanding Primary Care
- Previous experience of working within integrated healthcare organisations or teams
- Ability to work effectively as part of a team
- Ability to work alone without direct supervision when required
- Good time management and organisational skills, ability to prioritise effectively
- Ability to work under pressure whilst maintaining attention to detail and accuracy
- Ability to communicate effectively with members of the public and other healthcare professionals
- Ability to train other members of staff
- Ability to use Microsoft Office applications e.g. Word, Excel, Outlook
