Full Job Description

  • The Countess of Chester Hospital NHS Foundation Trust provides a primary care Medicines Optimisation service to 32 GP practices across Cheshire West Place.
  • We are looking for an enthusiastic, committed, and self-motivated pharmacist to join our team and support GP practices to prescribe in a safe and cost-effective manner.
  • Applicants must be registered as pharmacists with the GPhC and have relevant experience.

Participate in the provision and development of the primary care Medicines Optimisation service to GP practices across the Cheshire West footprint.
  • To support, promote and implement high quality, cost-effective, evidence based and safe use of medicines. This includes assisting practices to achieve prescribing targets, supporting the implementation of national and local guidance to improve patient care, and medication reviews.
  • To support practices to implement the Medicines Optimisation plan.
  • To develop, support & deliver the AMR action plan
  • To undertake monitoring of Non-medical prescribing
  • To undertake projects in GP practices.
  • To support NMP lead with governance process

The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.
