Full Job Description
- The Countess of Chester Hospital NHS Foundation Trust provides a primary care Medicines Optimisation service to 32 GP practices across Cheshire West Place.
- We are looking for an enthusiastic, committed, and self-motivated pharmacist to join our team and support GP practices to prescribe in a safe and cost-effective manner.
- Applicants must be registered as pharmacists with the GPhC and have relevant experience.
- Our friendly department provides an environment that supports learning, innovation, team working and encourages individuals to develop.
- You will have a 1 &1/2-day induction introducing you to our Trust, followed by a local induction plan.
- There are excellent holiday entitlements 27 days + bank holidays rising to 33 after 10 years’ service (pro rata) and the NHS has a competitive pension scheme.

- You must have a UK professional registration.

- To support, promote and implement high quality, cost-effective, evidence based and safe use of medicines. This includes assisting practices to achieve prescribing targets, supporting the implementation of national and local guidance to improve patient care, and medication reviews.
- To support practices to implement the Medicines Optimisation plan.
- To develop, support & deliver the AMR action plan
- To undertake monitoring of Non-medical prescribing
- To undertake projects in GP practices.
- To support NMP lead with governance process
