Full Job Description

We are looking to recruit a Lead Clinical Pharmacist within Stratforfd PCN to develop and lead our existing clinical pharmacist team and associated services within the Primary Care Network. The successful candidate will have a post-graduate diploma or higher degree in clinical pharmacy and will be an independent prescriber. They will perform both telephone and face-to-face medication reviews – including elderly patients, people resident in care homes and those with multiple and complex co-morbidities, whilst working towards local initiatives and helping support the teams KPI’s. The successful candidate will have a role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. The successful applicant will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, medicines optimisation, management of medicines on transfer of care and systems for safer prescribing. The candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews. To provide leadership to other clinical pharmacist and pharmacy technicians, take lead on the quality improvement and clinical guidance updates, as well as managing some aspects of the PCN DES requirements. An innovative mindset will be critical within the role, exploring new ways of working and driving both business and service improvements. Person specification: Professional Registration
  • Mandatory registration with the General Pharmaceutical Council
  • Membership of the Royal Pharmaceutical Society
  • Candidates should be able to align their skills with those highlighted in the Multi-Professional Framework for ACP
Qualifications
  • Relevant degree qualification
  • Independent prescriber
  • Completion of CPPE Pathway
Skills knowledge and experience
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence – based healthcare
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Excellent interpersonal, influencing and negotiating skills
  • Excellent written and verbal communication skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Good IT skills
  • Able to obtain and analyse complex technical information
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
  • Able to work under pressure and to meet deadlines
  • Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
