We have multiple exciting vacancies for Hypertension Prescribing Clinical Pharmacists based anywhere in the UK as these roles are home based. This is a unique opportunity which will encompass using innovative technology along with the Clinical Pharmacist using their specialised hypertension knowledge to improve patient outcomes and reduce stroke risk. Successful candidates will earn an extremely competitive salary which can be negotiated to match the candidates skills and qualifications, this can be discussed at interview stage. The post holder will be an experienced clinical pharmacist, who acts within their professional boundaries, to develop, and work within a team of pharmacists. They will work as part of a multi-disciplinary team taking responsibility for areas of chronic disease management. Role Requirements (Clinical Pharmacist) · Applicants MUST have previous general practice experience and an interest in hypertension · Applicants MUST have an independent prescribing qualification ·Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council ·Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role ·Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support ·Strong clinical leadership, communication and interpersonal skills ·Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role Role Responsibilities (Clinical Pharmacist) ·To contribute to improve health outcomes in long term conditions notably hypertension as a member of the multidisciplinary primary care team ·Manage, up-titrate if clinically appropriate suitable hypertension patients in conjunction with the technology provided. ·Prescribing of hypertension caseloads aligned with national and local guidelines ·To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to also provide evidence-based education and training for colleagues in partnership with the practice Company Core Prescribing Solutions is an award winning dynamic fast-paced healthcare organisation with over 20 years’ combined experience in the NHS, primary care, health informatics and the pharmacy sector.
