An excellent opportunity has arisen for a highly motivated specialist heart failure pharmacist with a broad range of clinical experience, to join the innovative integrated heart failure team at Imperial College Healthcare Trust (ICHT). The service delivers care across three acute hospital sites as well as in the community across the boroughs of Hammersmith and Fulham, Central and West London.
Applicants should have experience in delivering specialist pharmacist led clinics and demonstrate the ability to appropriately recommend, substantiate and communicate therapeutic options for patients with heart failure in line with latest evidence as well as National and local guidelines. Youmustbe skilled in clinical examination, assessment and independent prescribing for heart failure patients.
The role includes all aspects of heart failure care providing highly specialist clinics, education, training and clinical advice for the heart failure MDT and stakeholders across care settings. As a senior member of the heart failure team, you will be expected to lead on service development including audit, writing/reviewing policies and guidelines and participate in research.

- To lead, deliver, develop and evaluate specialist pharmaceutical care to patients in the heart failure service in accordance with the objectives set between the Heart Failure Nurse Consultant and the Senior Lead Pharmacist for Cardiovascular.
- To be a role model in the field of heart failure and provide a consistently high pharmaceutical service for patients in line with Imperial corporate objectives.
- To be a member of the heart failure multidisciplinary team, and deliver care to patients with heart failure.
- To offer expert advice on medication and pharmaceutical issues to the heart failure team and other health care professionals.
- To ensure patients’ pharmacological management plans and the heart failure medicines portfolio are safe, within manufacturers’ licenses or accepted clinical use, comply with all current legislation and are in line with local and national guidance.
- To be involved in education and training to the multidisciplinary team including service development and academic involvement.
