Remote Prescribing Pharmacist.
We are a dynamic, friendly and high achieving multi-sited London NHS GP practice with a strong team.
We are looking for a full-time enthusiastic and motivated experienced pharmacist to work in our busy GP Practice in Fulham. The successful candidate for this position should have a prescribing qualification and will be provided full Primary Care training by the current senior pharmacist and will be expected to achieved the following during the role:
To do telephone consultation for medication reviews and chronic disease management including Asthma reviews, blood pressure reviews and Diabetes medication review dealing with high HBA1C for uncontrolled Diabetics.
To ensure that prescriptions are up-to-date, revised and changed according need patient needs or changing condition acting promptly on requests from outside agencies. Prescriptions would need updating according to the prescribing policy.
· Act as a part of call for all medicines related issues within practice and contribute pharmaceutical expertise to clinical decisions
· To support new ways of working and workflow redesign to successfully embed pharmacists within general practice
· Provide professional specialist medicines management advice to GPs, practice nurses, community pharmacists, community nurses and other health professionals to promote the safe, effective, and high quality, cost effective use of medicines in all prescribing decisions.
· To identify, develop and promote quality improvements in medicines management across community pharmacy & GP practices ensuring that best practice is adopted
· To contribute to the development and implementation of the practice prescribing strategy and medicines management initiatives
· To carry out prescribing audits as required
· Attend and participate in any Practice meetings remotely
· At all times there will be a need to maintain accurate computerised records
· Communicating effectively with patients and carers, and recognising their needs for alternative methods of communication
· To adhere to all practice policies procedures and guidelines
· To comply with the standards set out in the contract of employment
· To undertake any other duties as appropriate to the role and in the detailed Job description.
Qualifications and Skills
· Degree in Pharmacy, MPharm and prescribing qualification.
· Excellent written and verbal communication skills
· Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences e.g. patients
· Good IT skills
· Able to obtain and analyse complex technical information
· Able to work under pressure and to meet deadlines
· Produce timely and informative reports
· Work effectively independently and as a team member
· Self-motivation
· Adaptable
Job Type: Part-time up to 25 hours per week
Salary: starting £55,000 /year FTE (FTE = 37.5 hours)
