Are you a clinical pharmacist looking to work within a team environment with peer support and excellent training opportunities? An exciting opportunity has arisen within Torridge Health Primary Care Network for a full-time Clinical Pharmacist. The successful clinical pharmacist candidate will play an important role within our pharmacy team and more widely in the PCN multi-disciplinary team. You will join our current team of 4 Clinical Pharmacists and 5 Pharmacy Technicians working from a central PCN hub with strong links with the individual practices. The FTE role has a competitive salary with 25 days holiday plus bank holidays, 1 week’s study leave and NHS Pension. We are looking for a motivated, enthusiastic and professional individual with a good clinical background, who is organised and confident to work independently and as part of a team, with the ability to prioritise workload. Applicants will be computer literate, have excellent communication skills, and be able to travel across our various Network sites for service provision. Knowledge of SystmOne and Ardens would be an advantage. We would welcome applications from pharmacists working in all sectors of pharmacy. The successful candidate will have a degree in pharmacy. A post-graduate diploma or higher degree in clinical pharmacy is desirable. A comprehensive programme of training will be provided as part of the CPPE Primary Care Pharmacy Education Pathway as well as full support from the other Network Pharmacists. The successful candidate is required to undertake the independent prescribing course to become a non-medical prescriber (if not held) and is expected to work across Network practices after initial training. Main duties of the job Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients within your current competence, using your expert knowledge of medicines for specific disease areas. Being responsible for the care management of patients with chronic diseases and undertake structured medication reviews to proactively manage people with complex polypharmacy. The post holder will provide primary support to general practice staff with regards to prescriptions and medication queries. They will help to support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice. The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. Providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities. Support the leadership team on ensuring person-centred medicines optimisation and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced services. Support patients to take their medications to get the best from them, reduce waste and promote self-care where possible. Support the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and other healthcare professionals. Contribute to clinical governance activities / clinical audit to ensure continual improvement of service delivery. Provide services to support the practices deliver QOF, DES, DMOS and IIF targets. Work to agreed standards, monitor and maintain the standard of care offered. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
