Clinical Pharmacist

We are currently expanding our Clinical Pharmacist bank posts to support a number of services being delivered across our frailty units. Successful candidates will be an integral member of a multi-disciplinary team, delivering transformed services to frail older people and working to ensure effective medicines optimisation for patients

• residing in care homes
• attending the Jean Bishop Integrated Care Centre
• residing in our frailty bedded units
• seen in primary care

You will be able to demonstrate excellent communication and organisational skills and have the confidence to engage and influence a wide range of healthcare professionals. You must be committed to the delivery of high standards in a patient focused service. Experience of performing Clinical Medicine Reviews is preferable but not essential. Applicants are welcome to apply from all backgrounds of pharmacy. You will receive professional support from our established Medicines Service team based in Hull.
