Job summary

The Minerva Pharmacy Team provide a vital and integral service to our practices. We have an existing team of 5 Clinical Pharmacists and 2 Pharmacy Technicians, and we are looking to further expand our team.  

Main duties of the job

The post holder will work as part of a multi-disciplinary team, primarily in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities. The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice/s. The post holder will provide clinical leadership in the GP Practice on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The postholder will be working towards or utilising their non-medical prescribing qualification. They will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.  

About us

Minerva Health Group Primary Care Network (PCN) consists of 4 GP Practices located within the City of Bath (Combe Down surgery, Newbridge surgery, Rush Hill & Weston surgeries and St Michaels surgery). The Practices have a history of working collaboratively to provide Primary Care services to their registered populations and have excellent relationships with the local GP Federation, B&NES Enhanced Medical Services (BEMS) who will be the employer of the Physician Associate. There is a strong team ethos across the PCN and there are already a number of staff working very successfully across our practices. With a patient population of 35K and growing, we are developing a Physician Associate team to complement our high level of patient- centred care. As a group of highly experienced training practices we would support successful candidates to develop their skills and can offer the Physician Associate Preceptorship programme to eligible candidates. We look forward to welcoming Physician Associate to our practice teams and further improve our services and help us manage and signpost patients through their journeys. BEMS is a not-for-profit Community Mutual Provider, whose members include local GPs and practices as well as staff and patients. It supplies NHS services that are free to all Bath & North East Somerset Patients. Successful applicant will have access to NHS Pension.
