Horton Rehabilitation Services: There are 4 wards including 2 high dependency wards, a managed access ward and a community rehabilitation ward. The successful applicant may have contact with patients or service users.   • To deliver an advanced comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care at Horton Rehabilitation Service
  • Promote personalised care and shared decision making with patients, including supporting the move towards self-management of medication.
  • Be a source of expert pharmaceutical advice to the MDT
  • To provide leadership and support to the pharmacy team providing the Medicines Optimisation service at Preston House.
  • Provide input into policy and procedures affecting the use of medicines at both local and trust wide levels.
Bank Advert for an 8a Pharmacist working in Mental Health Services An exciting opportunity has arisen for a motivated and enthusiastic Mental Health Pharmacist to join our bank to work at our MH Rehabilitation services, based at Horton, Surrey. You will need to have experience of working as a Mental Health Pharmacist and have excellent clinical, interpersonal and organisational skills
  • Ensure safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients. This includes regular review of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects (Medicines Act, Mental Health Act) of medicine supply and compliance with the Trust formulary.
  • Promote personalised care and shared decision making with patients, including supporting the move towards self-management of medication.
