The Pharmacy Department at Cambridge University Hospitals NHS Foundation Trust is ambitious and forward thinking, striving to ensure that the patient is always at the heart of everything we do. We are excited to be involved in the development of the CUH Virtual Ward programme, which aims to care for patients safely at the place they call home. In line with National NHSE ambitions and working with our Integrated Care System & Improvement & Transformation teams we are rolling out our virtual ward model. Virtual wards allow patients to receive the care they need at home safely and conveniently rather than being in hospital, using technology & digital platforms for us to monitor our patients across a range of clinical specialities. We are seeking a motivated and innovative Pharmacist to lead the virtual ward clinical pharmacy service as part of the wider MDT. The ideal post holder would be comfortable collaborating with clinicians, managers, nurses and others to ensure that pharmaceutical care of virtual ward patients with a range of clinical conditions is safe and optimised. You will have the opportunity to provide pharmaceutical care to a wide range of patients and/or patient populations based on the evidence-base and/or best practice.
  • Provide pharmacy leadership to the Virtual wards
  • Liaise with the relevant senior pharmacy, medical and nursing staff, other allied health professionals (AHPs) and managers to promote safe, evidence based, safe and cost-effective use of medication in Virtual wards
  • Provide an advanced clinical pharmacy service to Virtual wards / clinics as part of the MDT;
  • Provide specialist pharmacist advice and services to the Virtual wards, the specialist MDT and patients under their care

  • Managing the launch of clinical pharmacy services to the virtual wards
  • Working across boundaries with external partners in the ICS to transform patient care pathways and using your expertise to advise on novel virtual ward pathways for
  • Participation in the education and training of Pharmacy and MDT colleagues in relation to the care of virtual ward patients
  • Conducting audit, service evaluation and research into this new model of care.
