Advanced Clinical Pharmacist – UCR & Virtual Frailty Wards Lead (West)

We have an exciting opportunity for an enthusiastic, self-motivated and experienced pharmacist to work with our virtual frailty wards team across West Berkshire. This new post has been created for a pharmacist to lead and develop a high quality clinical pharmacy service for community patients with complex needs and long term conditions within the virtual frailty wards. The post offers an excellent opportunity for a dynamic, clinically experienced pharmacist to use his/her advanced clinical knowledge and experience within the older / frailty speciality. We will support you in your professional development, to think creatively, and lead service development within your area of responsibility. This role offers tremendous variation and many opportunities to engage and work collaboratively across the Trust. The successful candidate will work closely with colleagues in the UCR and Virtual Frailty Wards team to ensure pharmacy is appropriately provisioned for and is in line with all legal and professional aspects. There are opportunities to practice as an Independent Non-Medical Prescriber so we will support you in your professional development, including gaining the Independent Prescriber qualification.
