Lead Pharmacist for Community Services

An exciting opportunity has arisen at East Cheshire NHS Trust for an experienced and highly motivated individual to lead and develop the pharmacy service provided to our patients in Community Services within our innovative and friendly clinical pharmacy team. This will include virtual wards, as part of the wider MDT. East Cheshire NHS Trust provides a comprehensive range of acute and community-based services to a large population, in addition to a number of hospital services in partnership with other local trusts and private providers. By bringing acute hospital and community services closer together, our aim is to ensure that patients receive the best care in the right place. In this new post, you will work collaboratively with partners across the system to support the delivery of pharmaceutical care of virtual ward patients with a range of clinical conditions. You will ensure this is safe and optimised. You will also provide a high level of medicines governance and strategic support to the MDT. This is an exciting time to join the team as we are expanding our clinical pharmacy provision and implementing digital technologies including electronic prescribing and medicines administration (EPMA) systems in the next 2-3 years.
